Healthy Activities
Eaton is committed to your health as we continue to offer Healthy Incentives—a program that encourages you to focus on your health and wellbeing and find ways to reduce health risks. You have three actions you and your enrolled spouse/domestic partner can take: Know Your Numbers (complete a biometric screening and complete the Health Survey) and the Be Tobacco Free certification.
To get started, visit Rally or use the Rally app to complete your Healthy Activities from the home page. Your spouse/domestic partner must create his/her own login, using your Employee ID, including the E and leading zeros. (If you and your spouse/domestic partner are both Eaton employees, use the Employee ID of the person who enrolled in the medical plan.)

Know Your Numbers – Health Survey and Biometric Screening
To complete your Know Your Numbers Healthy Activities, complete a biometric screening AND take the Health Survey. Sign up for a screening and access the health survey from the Rally home page or from your Available Activities shown under the Rewards tab > Employer Rewards.
To schedule your screening, click “Schedule your Screening” on the Rally portal and choose from the following options:
- Sign up for your location’s on-site screening;
- Sign up for a Quest Patient Service Center (PSC) screening; or
- Share the Provider/Physician Result Form with your doctor and submit your test results completed by your doctor (including all measurements and the screening date) to Quest via fax or upload. Make sure to keep a copy of the form and the fax/upload confirmation
To complete the survey, click “Complete Survey” and allow 10 minutes to answer all sections of the survey to determine your Rally Age.
Be Tobacco Free
To earn your Healthy Activity, certify on Rally that you have been tobacco-free for four months, or complete a tobacco cessation program.
Even if you’ve never used tobacco, you need to certify each year that you are tobacco-free to receive the Be Tobacco Free credit, and to qualify for lower non-user rates for Eaton employee optional life insurance (where available) and the critical illness voluntary benefit.
If you are not tobacco-free, complete the Quit Tobacco program by talking to a personal coach or completing the online program. Either way, plan accordingly — you can only do one session per week, so it will take at least six weeks to complete the program. Don’t forget, you must complete all sessions by the deadlines to receive credit. Note: Enrolling in the Quit Tobacco program does not qualify you for lower non-user rates for employee optional life insurance (where available) or for the critical illness voluntary benefit.
