The program provides support through virtual sessions for depression, anxiety and stress that often accompany chronic medical health issues like diabetes, cancer or cardiac conditions. Eligible members will receive outreach if they qualify for this program, and you may also call the program and speak with a licensed clinical social worker or coach.
Treatment includes eight weeks of coordinated care using a whole-person approach. If you are enrolled in the program, you can monitor your progress toward meeting all the participation criteria using the online portal. You’re encouraged to visit the site frequently to keep on top of the activities you should be completing and ensure that your information is up to date. The site also includes links to other helpful tools and resources for Behavioral Health.
