Your wellbeing is important, so take some time to think about your family’s health care needs for 2025. You’ll want to consider your paycheck contributions, deductibles, out-of-pocket maximums, and your Health Savings Account. If you don’t enroll, most of your 2024 elections will carry over automatically into 2025—but it’s important to review and make your elections carefully each year and choose the right benefits for your unique needs. Be sure to log on to NetBenefits during Annual Enrollment as there are certain benefit elections that will not carry over and must be made each year.

- MyChoice Medical Plan Details – Plan details including list of copays. Eaton is switching to a further simplified version of the Surest MyChoice plan. The MyChoice plan will no longer have coverages requiring activation. It will be a variable copay only plan.

Use Your Benefits
- Visit whyuhc.com/Eaton, the UHC website, to explore coverage options or price medications in advance of the new year.
- Visit join.surest.com/Eaton (access code Eaton2025) to check out the MyChoice plan details.
- If you are already enrolled in Eaton coverage, use Rally or the Surest app to access current claims, balances, ID cards and other benefits information.
2025 Plan and Contribution Changes
Due to rising healthcare costs, contributions will increase slightly for 2025. Your 2025 biweekly contributions are shown below. To calculate the weekly contribution amount, divide the biweekly contribution by two. Please reference this chart to see a side-by-side comparison of all three medical plans. We are happy to share that there will be no changes to dental or vision benefits or costs. Voluntary benefits including Accident Insurance, Hospital Indemnity Insurance and Critical Illness Insurance will have 10% rate reductions and plan design enhancements for 2025. Healthy Incentives
Keep more money in your paycheck by completing your Healthy Incentives. You can earn up to $1,800 towards your medical plan contributions, putting more money in your pocket at the end of the day. Receive rewards for living healthier!
Click here to visit Rally to complete your activities and earn valuable medical plan contribution credits for 2025! You must complete all actions by the end of the year to receive these credits. If you complete the actions by December 31, you’ll receive credits starting March 1 of the following year.
Flexible Spending Accounts (FSAs) are moving to Fidelity
Our FSA vendor is changing from HealthEquity to Fidelity. There is no action required on your part to transition your account(s) to Fidelity. New cards will be issued, and further communications will be coming for those who have accounts and/or make elections during Annual Enrollment.
The accounts included in this transition are:
- Health care flexible spending account (FSA) (Previously referred to as the Health Care Reimbursement Account/HCRA)
- Limited purpose flexible spending account (LPFSA) (Previously referred to as the Dental/Vision Reimbursement Account/DVRA)
- Dependent care flexible spending account (DCFSA)
- Health reimbursement arrangement (HRA)
You will be mailed a new debit card as part of this transition to Fidelity. This special-purpose Visa® debit card provides an easy and automatic way to pay for eligible expenses. The same card will give you access to all spending account funds you have with Fidelity. When you use your debit card to pay for eligible expenses, the amount is automatically deducted from the appropriate spending account. The card will be mailed to you in December 2024 for use starting Jan. 1, 2025. You also can request additional cards for eligible family members once you have received and activated your card.
FSA eligible expenses and claim submission deadlines will not change due to the transition to Fidelity. Please submit any 2024 claims to HealthEquity by March 31, 2025.
Purchase Additional Vacation Days
Are you planning a life-changing trip around a milestone event? Need some extra R&R this coming year? Annual Enrollment is the time to purchase additional vacation days for 2025. Eaton understands that time away from work is essential for the health of its employees. Work/life balance can look different for everyone, which is why we offer up to one work week of additional vacation time for purchase.
You are eligible to participate in the Purchased Vacation Program if you work at least 20 hours per week. New hires are not eligible for this benefit in the first year. Visit the Employee Center for more information.
Note: If you are represented by a recognized collective bargaining agent, you are eligible for Purchased Vacation Program only if stated in the collective bargaining agreement.
Medical Plan Program Enhancements
We are proud to offer you three new programs to better care for your medical needs and potentially save you money.
Level2 Diabetes Management is designed to help type two diabetics better manage their condition using Continuous Glucose Monitors (CGMs) and integrated virtual clinical support. Every member that enrolls in Level2 receives a free Continuous Glucose Monitor (CGM) and personalized treatment plans with support from their virtual care team via the Level2 App. Starting January 1, 2025 you can go to mylevel2.com/care or call 844-302-2821 to enroll. - Color for cancer screenings. Keeping up with your cancer screenings is a critical—and proven—way to catch cancer earlier. In our commitment to health and prevention, Eaton has partnered with Color to support you with an easier way to coordinate and manage cancer screenings. Get free at-home cancer screening kits, easy access to in-person services like mammograms and colonoscopies, private genetic testing, and support from a team of cancer experts with Color. Voluntary benefits enhancements
Voluntary benefits including Accident Insurance, Hospital Indemnity Insurance and Critical Illness Insurance will have both 10% rate reductions and plan design enhancements for 2025.
Accident, Critical Illness and Hospital Indemnity Insurance with new Plan Enhancements may help you pay for unexpected costs resulting from an accident, serious illness or hospital stay. MyChoice Plan (powered by Surest)
Powered by Surest, the MyChoice plan offers lower monthly contributions than the Enhanced plan. Your comprehensive coverage begins on day one with no annual deductible and no coinsurance, and you’ll be able to check online to find the total cost of your care and copays before heading to your provider. Visit join.surest.com/eaton (access code: Eaton2025) to preview costs for services at your current providers and search for services you may need in the future. New in 2025 for the MyChoice plan, you no longer need to activate flexible coverage for plannable procedures. In 2025, all procedures will only have copays, including those which had previously required activation. Shop for the procedure, compare options and costs, then choose what's right for you and your family. As a reminder, providers who are higher quality and more efficient will have lower copays.

Care.com
Care.com’s family care benefits are here to support you as you’re trying to manage it all while caring for those you love, including yourself. Eaton offers a Care.com membership, which includes the ability to post jobs and search and reach out to local caregivers 24/7. Search for care for children, adults, or pets, or find services such as housekeeping, tutors, and more. Also included is access to Care.com’s Backup Care services, which is vetted and subsidized adult care and childcare when your regular care is not available.
